6.1 Need for study :-
Ayurveda the science of life covers not only the knowledge of life but also deals about healthy living and longevity. It basically relates the life of an individual with universe. The human race has been in pursuits of remedies for its ailments since time immemorial.

Vagbhata explains that if a drug is found suitable still it should be examined to
determine its rasa, virya, vipaka, guna ,karma, prabhava, procedure of use, dosage, and type of patient for whom it is meant.
The drug Medasaka (Litsea glutinosa Lour.) which is available in Udupi and it has been successfully used by folklore practitioners both externally and internally in the management of fracture , joint disorders, diarrhoea, dysentry and as a general tonic.
Janusandhi (Knee joint) is the largest and most complex synovial joint of the body and is a great importance. Janusandhigata vata (Osteoarthitis knee) is a vata vyadhi dominated by pain, swelling and stiffness. This is prevalent nowdays because of modern life style and food habits.
The root bark, stembark and leaf are proved for its anti inflammatory and analgesic activity on experimental animals (Menon Saritha et al., 2007). By looking this preclinical results we planned to evaluate the effect of stembark and leaf in the management of Janusandhigata vata.
6.2. Review of Literature:
The drug Medasaka (Litsea glutinosa Lour.) is been used by various folklore practitioners in various ailments as per the information collected from Folklore Medicine Research Centre, SDMCA Udupi. There is no reference of Medasaka observed in Samhitha, Purana and Veda. Botanical description, properties and therapeautic uses of Litsea glutinosa mentioned in recent books (Kirtikar, K. R. & Basu, B. D., 1933, Nadkarni, K. M. 1979, Bhat, Gopalakrishna K., 2003 ).
A study on anthelminthic activity against earthworm and tapeworm by using essential oil from Litsea glutinos (Mishra, et al., 1979).
A study on hypotensive effecting anasthetized dogs in a dose of 125mg/ kg by using essential oil obtained from Litsea glutinosa (Kar, et al., 1970). Acarya Susruta has mentioned Janusandhi as kora sandhi among the 8 type of sandhi and also mentioned sandhigata vata.( Acarya Susruta). In Caraka Samhitha Janu pramana and the disease Sandhigata vata mentioned under Vatavyadi ( Acarya Agnivesa).
6.3 Objective of study

Research updates on the drug Medasaka (Litsea glutinosa Lour).

Clinical study to evaluate the efficacy of stem bark powder orally and external application of leaf powder paste in Janusandhigata vata (Osteoarthritis Knee).
7. Materials and Methods:-
7.1 Source of data:
-
Literary and update research information of Medasaka (Litsea glutinosa) will be
collected from the folklore and other literatures.

The plant material the stem bark and leaf of Medasaka will be collected in and around Udupi.

A minimum of 20 patient of age group above 30yrs fulfilling the inclusive criteria will be selected for the clinical study from the OPD/IPD of SDM Ayurvedic Hospital Udupi.
7.2 Method of collection of the data:
Clinical study

It is single blind clinical study with pretest and post test design.

Patient of either sex will selected randomly.

A special proforma containing details necessary for the subject will be prepared.

Necessary investigation will be carried out.
Materials used;
Oral administration;
-
Tablet: Prepared with stembark powder of Medasaka with hot water.
External application
-
Pradeha: Leaf powder paste is prepared with hot water and applied over the joint.
Duration & Dose;
-
Tab: 1tid with hot water orally ( 1tab =500mg).
Inclusion criteria.
-
Patient with Pratyatma lakshana of Janusandhigata vata.
-
Patient in the age group of above 30yrs.
Exclusion criteria;
-
Patient suffering from Sandhigata vata as Upadrava of Amavata , Vatarakta and Phiranga.
Assessment criteria :
The patient’s response will be assessed on Subjective and Objective parameters.

Pain.
-
Tenderness.
-
Swelling.
-
Tempearture.
-
Crepitus.
-
Range of Movement, will be assessed on the basis of standard parameters.
7.3 Investigations :-
Hb%, TC DC. ESR, RA factor.
X-ray knee.
Intervention:
Tablet prepared with stembark powder of Medasaka will be given with hot water
orally.
Leaf powder paste of Medasaka prepared with hot water (pradeha) for external
application.
7.4. Has ethical clearance has been obtained from institution in case of 7.3?
