YENİ BİR LAZER FLORESANS CİHAZININ OKLUZAL ÇÜRÜK TEŞHİSİ AÇISINDAN İN VİTRO OLARAK DEĞERLENDİRİLMESİ
IN VITRO EVALUATION OF A NEW LASER FLOURESCENCE DEVİCE FOR THE DETECTION OF OCCLUSAL CARIES

ÖZET
Son yıllarda, başlangıç halindeki okluzal çürüklerin teşhisinde kullanılmak üzere lazer floresans esaslı yeni bir cihaz (Di-agnodent) geliştirilmiştir, in vitro çalışmalarda bu cihazın ölçüm değerlerini bazı faktörler (diş yüzeyindeki renklenmeler, dişlerin saklama koşulları gibi) etkilemektedir. Bu çalışmanın amacı, başlangıç halindeki okluzal çürüklerin teşhisinde dişlerin kuru veya nemli olmasının Diagnodent ölçümleri üzerine etkisini incelemek ve Diagnodent cihazının güvenirliliğini histolojik olarak elde edilen bulgularla kıyaslamaktır.
Çalışmada, göz ile muayenede herhangi bir kavitasyon saptanmayan 71 adet yeni çekilmiş insan küçük ve büyük azı dişi kullanıldı. Diagnodent ölçümleri önce dişler nemli iken daha sonra hava spreyi ile kurutularak iki ayrı hekim tarafından yapıldı. Ölçümler iki hafta sonra aynı şartlarda tekrarlandı. Histolojik değerlendirme, dişler % 0.5' lik bazik fuksin içinde bekletildikten ve takiben okluzal fossanın ortasından geçecek şekilde mezio-distal yönde iki eşit parçaya bölündükten sonra steromikroskopta x 10 büyütmede yapıldı.
Çalışmanın sonucunda elde edilen bulgulara göre, dişler nemli iken yapılan Diagnodent ölçümlerinin kuru iken yapılan ölçümlere göre daha güvenilir olduğu gözlendi (p<0.05).

SUMMARY
In recent years, a new laser fluorescence device, Diagnodent was introduced into the dental practice for the detection of occlusal caries. According to the results of in vitro studies, it is reported that some factors (staining of teeth surfaces, sto-ring conditions of the teeth, etc.) effect the measurements of Diagnodent. The aim of this study was, to assess the accu-racy of Diagnodent in comparison to the histological findings and to quantificate the effect of wet and dry surfaces on Diagnodent measurements.
The material comprised 71 extracted human premolar and molar teeth with visually intact occlusal surfaces. Each occlusal surface was measured by two observers first when the teeth were wet and than after drying the teeth with air flow. The measurements were repeated after two weeks under identical conditions. For histologic examination, the teeth were stored in basic- fuchsin dye and than sectioned into two equal sections mesio-distally across their occlusal fossas. The sections were examined by steromicroscope under x 10 magnification.
The results of the study showed that; Diagnodent measurements made under wet conditions were more accurate, com-pared with the measurements made under dry conditions.
