Learning Site Agreement (LSA) in Place - “Approved” Community Partners



Message for Faculty:

  1. Please contact Stacy Becker x4963 if you have any questions regarding whether an agency or program is “approved.” Some community programs may be covered by currently “approved” agencies with the Learning Site Agreement (LSA) and Learning Site Self-Assessment (SSA) documents already established.

  2. This is a resource list for faculty use only; it is not designed to be directly distributed to students, due to careful relationships built by specific departments with the community partners.

  3. Agencies on this list may have their own required documents and may conduct their own screening/ background check and acceptance procedures.


Please note: HSU students are required to complete the following 4 forms for placements at all OFF campus, non HSU organizations only (these 4 forms are now recommended, not required, for HSU direct/ ON campus programs): The 1) HSU Student Learning Plan, 2) HSU Participation Guidelines, 3) HSU Student Time Log, and 4) HSU Release of Liability forms are required of HSU students for Service Learning and Academic Internship off-campus placements for academic credit. HSU Programs


Direct HSU Programs/Departments

(No LSA required)

In general, the Learning Site Agreement (LSA) is NOT required for direct HSU programs; please contact the CSLAI if you have any questions. The Learning Plan, Participation Guidelines, Student Time Log, and Release of Liability forms are recommended for HSU students serving with ON CAMPUS HSU programs for academic credit (see examples below). Please note: students serving in HSU Programs for course credit MUST be supervised by professional (non-student) staff.

Government Agencies – Not Approved
Please contact Stacy Becker (x4963 spb1@humboldt.edu) if you have any questions or are overseeing a clearly identified student wishing to serve as an Academic Intern or via a Service Learning course at the following agencies. We have made MANY efforts (in some cases over a period of years) to establish the Learning Site Agreement paperwork with these larger state/national agencies, which have been unable to resolve legal contractual issues. We can begin the process again when HSU students are clearly identified as wishing to serve in Academic Internship or Service Learning positions at sites under these agencies. Meanwhile, the CSU Chancellor’s Office may be working on CSU wide Agreements with some of these agencies as of Spring 2015.
