Anti-Amoxicillin Immunoglobulin E, Histamine-2 Receptor Antagonist Therapy and Mast Cell Activation Syndrome Are Risk Factors for Amoxicillin Anaphylaxis

ABSTRACT

BACKGROUND: β-Lactam antibiotics (mainly amoxicillin, AX) are the drugs that most frequently induce systemic drug allergy reactions.

OBJECTIVE: We attempted to identify the risk factors associated with systemic reactions to AX.

METHODS: All patients who were referred to our department for suspected hypersensitivity reactions to AX over a 6-month period were evaluated for anti-AX immunoglobulin E (IgE) levels and skin-test positivity for β-lactams. Age, sex, concomitant diseases, therapies, total IgE, serum tryptase levels and signs and symptoms suggesting mast cell activation syndrome (MCAS) were analyzed in relation to the severity of the reaction in accordance with the Mueller classification.

RESULTS: Sixty-seven patients were selected: 39 with mild reactions such as cutaneous or gastrointestinal symptoms (grades I and II) and 28 with severe systemic reactions (grades III and IV). Anti-AX IgE levels and total IgE were significantly higher in severe reactions than in mild ones (p < 0.00005, p = 0.0037). Treatment with histamine-2 receptor antagonists (anti-H2) was significantly related to severe reactions (p = 0.007). No significant correlations were found between the severity of the reactions and dyslipidemia or levels of angiotensin-converting enzyme and tryptase.

CONCLUSION: Anti-AX IgE levels were the most significant immunological parameter distinguishing patients who presented with severe reactions to AX and those with mild reactions. Higher values of total IgE, the use of gastroprotective drugs and signs and symptoms suggesting an MCAS significantly increased the odds ratio of having a severe reaction. The risk of serious adverse reactions to AX increased in older patients and in males, but this trend was not significant.



ÖZET

GİRİŞ VE AMAÇ: β-laktam grubu antibiyotikler (özellikle amoksisilin, AX) sistemik ilaç alerjisi reaksiyonlarına en sık neden olan ilaçlardır. Bu çalışmada, amoksisiline bağlı sistemik reaksiyonlar ile ilişkili risk faktörlerini belirlemek amaçlanmıştır.

METOD: Altı aylık süre içinde AX şüpheli aşırı duyarlılık reaksiyonları için kliniğe sevk edilen tüm hastalar anti-AX immunglobulin E (IgE) düzeyleri ve beta-laktamlar için deri testi pozitifliği açısından değerlendirilmiştir. Yaş, cinsiyet, eşlik eden hastalıklar, tedaviler, total IgE, serum triptaz seviyeleri ve mast hücresi aktivasyon sendromu (MCAS) düşündüren belirtiler ve semptomlar Mueller sınıflandırmasına uygun olarak reaksiyon şiddeti ile ilişkili olarak analiz edilmiştr.

SONUÇLAR: Otuz dokuz deri ve gastrointestinal semptomlar gibi hafif reaksiyonları olan (sınıflarda I ve II) ve 28 ciddi sistemik reaksiyonları olan (derece III ve IV) toplam 67 hasta seçilmiştir. Anti-AX IgE ve total IgE düzeyleri şiddetli reaksiyonları olanlarda hafif olanlara göre anlamlı derecede yüksek bulunmuştur (p <0.00005, p = 0.0037). Histamin 2 reseptör antagonistleri (anti-H2) ile tedavi şiddetli reaksiyonlar ile ilişkili bulunmuştur (p = 0.007). Dislipidemi, anjiyotensin dönüştürücü enzim ve triptaz seviyeleri ile reaksiyonların şiddeti arasında anlamlı bir korelasyon gösterilmemiştir.



SONUÇ: Anti-AX IgE düzeyleri AX ilişkili reaksiyonlar ile başvuran hastalarda hafif ve şiddetli reaksiyonları ayırt etmede önemli bir immünolojik parametre olarak bulunmuştur. Yüksek total IgE düzeyleri, mide koruyucu ilaçların kullanımı ve MCAS düşündüren semptomlar ciddi reaksiyon oranını arttırmaktadır. AX’e bağlı ciddi advers reaksiyon riski yaşlı hastalarda ve erkeklerde artmaktadır, ancak bu eğilim anlamlı değildir.

Selective immediate responders to amoxicillin and clavulanic acid tolerate penicillin derivative administration after confirming the diagnosis

ABSTRACT

BACKGROUND: An increasing number of patients show immediate selective hypersensitivity reactions to clavulanic acid (CLV) and amoxicillin (AX), probably due to their increased prescription. The maintenance of this response should be established.

OBJECTIVE: To assess that the immediate hypersensitivity selective response to AX or to CLV is maintained after repeated administration of penicillin G (PG)/penicillin V (PV) and AX.

METHODS: Patients with proven immediate hypersensitivity to AX (Group A) or CLV (Group B) were included. Diagnosis was performed using skin tests with major and minor determinants of PG (PPL/MDM), AX and CLV and by drug provocation test (DPT) if required. Selectivity was established by confirming tolerance to PG/PV (Group A) and to PG/PV and AX (Group B). The maintenance of the selective response was verified by repeating DPT, 15 days after the initial investigation, with the same procedure.

RESULTS: Of 51 patients, 78% belonged to Group A and 22% to Group B. Most had anaphylaxis. In Group A, 72% were skin test positive; 28% required DPT. In Group B, 63% were skin test positive; 37% required DPT. Only two AX-selective cases developed positive responses after re-provocation with PG/PV. No cases selective for CLV developed a positive response to PG, PV or AX.

DISCUSSION: The selective response to AX appears consistent, and a response to penicillin determinants only develops in a minority of cases. For the case of CLV, the selective response appears not to be modified by exposure to penicillin determinants, meaning that patients with CLV allergy can take penicillin derivatives safely.

GİRİŞ VE AMAÇ: Bu çalışmada, tekrarlayan penisilin G (PG)/penisilin V (PV) ve Amoksisilin (AX) uygulamaları sonrası seçici (ortak beta laktam halkası dışındaki yan zincire karşı gelişen reaksiyonlar) amoksisilin (AX) veya klavulonik asit (CLV) erken aşırı duyarlılık reaksiyonlarının devamlılığını değerlendirmek amaçlanmıştır.

METOD: Çalışmaya kanıtlanmış AX (Grup A) veya CLV (Grup B) erken aşırı duyarlılık reaksiyonu olan hastalar dahil edilmiştir. Tanıda PG (PPL/MDM) majör ve minör determinatları, AX ve CLV ile deri testi ve gerek görülürse ilaç provokasyon testi (DPT) kullanılmıştır. Seçicilik PG / PV (Grup A) ve PG/PV ve AX (Grup B) toleransının doğrulanması ile belirlenmiştir. Grup A amoksisilin duyarlı ve PG/PV tolerant, Grup B ise klavulonik asit duyarlı PG/PV ve AX toleranttır. Seçici cevabın devamlılığı 15 gün sonra DPT tekrarı ile doğrulanmıştır.

SONUÇLAR: 51 hastanın, % 78’i A Grubu %22’si B Grubudur. Grup A hastalarının % 72’sinde cilt testi pozitifliği saptanmıştır ve % 28’inde DPT gerekli olmuştur. Grup B'de ise % 63’ünde cilt testi pozitif bulunmuş; % 37’sinde DPT gerekmiştir. Sadece iki AX-selektif vakada PG / PV ile yeniden provokasyon sonrası pozitif tepkiler gelişmiştir. Selektif CLV duyarlılığı olan vakalarda PG, PV veya AX ile reaksiyon gelişmemiştir.



SONUÇ: AX selektif reaksiyonlar tutarlı görünmektedir ve vakaların çok az bir kısmında penisilin ile reaksiyon gelişmiştir. Selektif CLV allerjisi olan hastalar güvenle penisilin türevlerini alabilir görünmektedir.

Factors associated with negative histamine control for penicillin allergy skin testing in the inpatient setting

BACKGROUND: Identification of factors adversely affecting the utility of allergy skin testing is important in optimizing patient care. Inpatient penicillin skin test data from 1997 through 2007 demonstrate that up to 20% of attempted penicillin skin tests are indeterminate owing to a negative histamine test response, despite exclusion of H1 antagonists. Critical illness, vasopressors, steroid use, and psychotropic medications have been postulated to influence outcomes, but large studies are lacking.

OBJECTIVE: To identify factors associated with a negative histamine test response for the inpatient setting.

METHODS: Fifty-two cases were identified with a negative histamine response after penicillin skin testing in the absence of antihistamine therapy for 72 hours before testing. One hundred twenty-five controls with a normal histamine response were randomly selected from same population. Independent variables assessed included stay in the intensive care unit (ICU), skin color, diabetes, age, use of vasopressors, H2 blocker, steroids, other immunosuppressive drugs, thyroid replacement, proton pump inhibitors, diuretics, 5 categories of psychotropic medications, and amiodarone.

RESULTS: Mean age was 68 years for cases vs 60 years for controls (P = .002). Bivariate analysis showed ICU stay was more frequent in cases than in controls (73.1% vs 33.6%, P < .001). Regression analysis yielded odds ratios (ORs) of 8.18 (95% confidence interval 3.22-20.76) for ICU status, 3.76 (1.30-10.92) for systemic corticosteroids, and 4.90 (1.17-20.62) for H2 blockers as associated with lack of histamine response. For every additional year in age, there was increase in the OR of 1.04 (1.01-1.07).

CONCLUSION: Regression analysis supports ICU stay during skin testing as associated with a high OR for a negative histamine response independent of age. Systemic corticosteroids, H2 blockers, and older age are associated with a significant OR for a negative histamine response. This is one of largest studies on factors associated with a negative histamine response for the inpatient setting and has significant implications for clinical practice.

SONUÇLAR: Olguların ortalama yaşı 60, kontrollerin ise 68 idi (p = .002). Yoğun bakım kalış oranı olgularda kontrollere göre daha yüksekti. (%73.1 ve % 33.6, p <.001). Regresyon analizi sonrası histamin yanıtsızlığı ile ilişkili olarak yoğun bakım yatışı için odds oranı 8.18 (95% (95% confidence interval 3.22-20.76), sistemik steroid kullanımı için odds oranı 3.76 (1.30-10.92), H2 bloker kullanımı için odds oranı 4,90 olarak bulunmuştur. Yaşta, her ek yıl için odds oranında 1.04 (1,01-1,07) artış olmuştur.



SONUÇ: Yaştan bağımsız olarak yoğun bakımda kalma negatif histamin yanıtı ile ilişkili bulunmuştur. Sistemik kortikosteroidler, H2 blokerleri ve ileri yaş önemli bir Odds oranı ile negatif histamin yanıtı ile ilişkilidir.
