Do you clearly understand that although medicine and medical care is paid by the rescue, the foster home is normally expected to pay for a foster dog’s food, treats, collars, leashes, toys, beds, crates, grooming, etc. while in your care?

In the event you adopt, do you clearly understand that it may cost approximately $1200 a year for veterinary care, grooming and proper food and that the lifespan of a small dog may reach 15 years or more?

By signing this application I acknowledge that all my statements are true and correct. I have never been accused or convicted of animal neglect or cruelty. I understand that any dog I may foster, transport, or pull from a shelter for Tzu Zoo Rescue is owned by Tzu Zoo Rescue (although I do have the option to adopt the dog). I also state it is my intention to give any dog I may adopt love and respect, welcome this dog as a part of my family and into my home, and be dedicated to working thru any problems with this dog.
