REPRESENTATION: [ ] By signing this Entry Form, the Cowpens Idol contestant and/or contestant parent or guardian, acknowledges that the Mighty Moo Festival is NOT responsible for any prizes promised or offered by anyone, except the chance for the qualified contestants to compete in the scheduled Cowpens Idol Contest for a win and 1st place will receive a trophy and gain a spot to perform during the intermission at the street dance on June 20, 2015. Second place will receive a gift certificate. I, ________________________________, grant to the Mighty Moo Festival the right to utilize the act's name, likeness, image, voice, appearance, and performance in perpetuity worldwide for advertising, trade or any other lawful promotional purposes whatsoever, in any and all media now known or thereafter developed, without compensation. I grant the Mighty Moo Festival the right to broadcast, market, sell and otherwise distribute my name, image, likeness, voice, appearance and performance, either in whole or in parts for commercial, non-commercial television, close circuit exhibition, home video distribution, internet, or any other purpose that the Mighty Moo Festival in their sole discretion may determine.
