Conflict of Interest Disclosure User Guide (v.2013)
Conflict of Interest (COI) Disclosure is an application used by investigators at the Marshfield Clinic Research Foundation (MCRF) to disclose interests as required by the Investigator Conflicts of Interest in Research policy.
How To


View Training

  1. Open Conflict of Interest Disclosure through the email link or http://biz1-lc:8080/coi/Pages/Home.aspx.

  2. Click Training and then click Begin Training.

  3. Training is required every four years and can be reviewed at any time by clicking the Training.

New Annual Disclosure

  1. Open Conflict of Interest Disclosure through the email link or http://biz1-lc:8080/coi/Pages/Home.aspx.

  2. Click New Disclosure and then click Annual

  3. The disclosure form will open.

  4. If the Annual button is not available, you have already started or completed your annual disclosure.


New Transactional Disclosure

  1. Open Conflict of Interest Disclosure through the email link or http://biz1-lc:8080/coi/Pages/Home.aspx.

  2. Click New Disclosure and then click Transactional

  3. The disclosure form will open.

  4. If the Transactional button is not available, you have not completed your annual disclosure.


Open In Progress Disclosure

  1. Open Conflict of Interest Disclosure through the email link or http://biz1-lc:8080/coi/Pages/Home.aspx.

  2. Click In Progress Disclosure and then select a disclosure from the list.


View Disclosure History

  1. Open Conflict of Interest Disclosure through the email link or http://biz1-lc:8080/coi/Pages/Home.aspx.

  2. Click Disclosure History and then select a disclosure from the list.


Save In Progress Disclosure

  1. The application automatically saves each questions when you click Next

  2. If you close your browser before clicking next, the current answers will not be saved.


Add Interests

  1. A window opens based on the answer selected while completing the disclosure form.

  2. Enter details.

  3. Click Save.

  4. Click Add Entry to enter additional interests.


Modify Interests

  1. To modify an existing interest Click Modify Entry.

  2. If more than one interest has been added, click on the interest you wish to modify.

  3. Click Modify.

  4. Modify details.

  5. Click Save.


Delete Interests

  1. Click Modify Entry.

  2. Click the tab to select an interest.

  3. Click Delete.


Back Button

  1. At times a back button may be available. Clicking the back button takes the user to the previous question.

  2. Because the questions are dynamic, using the back button may require the user to re-enter information.

  3. It is advisable to only use the back button when absolutely necessary.


Add Attachments

  1. Within a question section, click Browse.

  2. Locate and select the document and then click Open.

  3. Click Upload.


View Attachments

  1. Click View Attachments.

  2. Click Download next to the appropriate file.

  3. In the File Download window, click Open to view the document.


Delete Attachments

  1. Click View Attachments.

  2. Click Delete next to the appropriate file.


Completing a Disclosure without Reviewing

  1. After completing all tabs, locate the Submit button at the bottom of the screen.

  2. To skip viewing a Summary of your disclosure Click Submit.

  3. On the Disclosure Authorization page, enter your Marshfield Clinic password and Click Submit.

  4. Click the Summary Button and review details on the Disclosure Summary page.

  5. Click to print a copy for your records.

  6. Click Submit.

  7. On the Disclosure Authorization page, enter your Marshfield Clinic password.

  8. Click Submit.

  9. A confirmation message and email are received upon successful completion.

Completing a Disclosure with Review

  1. After completing all tabs, locate the Summary button at the bottom of the screen.

  2. Click the Summary Button and review details on the Disclosure Summary page.

  3. Click Submit.

  4. On the Disclosure Authorization page, enter your Marshfield Clinic password and click Submit.

  5. A confirmation message and email are received upon successful completion.


Printing a Disclosure

  1. Open Conflict of Interest Disclosure through the email link or http://biz1-lc:8080/coi/Pages/Home.aspx.

  2. Click Disclosure History and then select a disclosure from the list.

  3. Click the Printer Friendly icon

  4. A new window will open with a Printer Friendly version of your disclosure

  5. Click Print this page

  6. Select the printer and click Print.





Helpful Tips


  • The COI Application is designed to work with Internet Explorer 8

  • The disclosure form may be saved in progress. You can return to the application at a later date to complete your disclosure form.

  • Unless specified, dates must be entered in the following format: mm/dd/yyyy.

  • Do not enter non-numeric characters in number fields. Example: If you enter a dollar amount, enter as 5000 not $5,000.
